Dear Sir/Madam: Thank you for your interest in the activity monitor. When you have received the unit, please read the instructions carefully. The unit has to be worn at all times, including sleep, and removed only when showering, bathing or swimming, as it is not water-proof, and can be damaged by contact with water.

Before we schedule an appointment for a consultation, we need to obtain some basic information. Please follow these three steps:
1- Complete the Registration form to receive your ID number
2- Complete the Enrollment form (Child or Adult version)
3- When finished, please Contact us with your ID number
After reviewing your answers, our staff will contact you to schedule an appointment.

Instructions for Activity Monitoring
Enclosed please find an activity monitor programmed for your child. Please remember: • The belt with a pouch containing the monitor has to be worn around the waist at all times, including bedtime and sports. • The only time when you are to remove the monitor is when in the water (swimming, showers/baths, splashing water, etc.). • Bedtime and rising time should be always marked on the enclosed sheet.
